ELIGIBILITY: To participate in this Sweepstakes, you must be a legal resident of the 50 United States and D.C. and be at least 18 years of age at time of entry. Employees and members of the board of directors of CRN Foundation and CRN, advertising and promotion agencies, vendors, and immediate family (defined as spouse, IRS dependent, or biological, adoptive or step-mother, father, sister, brother, daughter or son, and each of their respective spouses) of, or who reside in the same household as, any person in any of the preceding categories, are not eligible to participate. Void outside the 50 United States and D.C. and where prohibited.

HOW TO ENTER: To enter the Sweepstakes, visit www.lifesupplemented.org, and follow the links to participate in America's Wellness Challenge. Complete the Sweepstakes entry form in its entirety, check the box to agree to and accept the terms of the Official Rules and submit. Next, take the My Wellness Scorecard quiz, and upon reaching the My Wellness Scorecard results page, you will be automatically entered into the Sweepstakes. In the event of a dispute, entries will be deemed made by the "authorized account holder" of the e-mail address submitted at the time of entry. The "authorized account holder" is deemed the natural person who is assigned to an e-mail address by an Internet access provider, service provider or other online organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Entries made by any other individual or entity that is not the person named on the submitted entry, or made in any manner inconsistent with these Official Rules, will be declared invalid and disqualified from this Sweepstakes.

AMOE (Alternate Means of Entry): To enter the Sweepstakes without completing the My Wellness Scorecard, hand print your name, complete address, city, state, zip code, birth date and email address on a 3" x 5" card and mail it to: CRN Foundation "America's Wellness Challenge" Entry, 625 Panorama Trail (Building 2, Suite 100), Rochester, NY 14625-2437. Mailed entries must be postmarked no later than 02/28/10 and received no later than 03/10/10. Each mail-in entry must be mailed in a separate outer envelope. No copies, facsimiles or mechanical reproductions will be accepted. Limit one mail-in entry per post-marked envelope. Partially completed and/or mechanically reproduced entries or rubber-stamped entries will be void and ineligible. All entries received become the sole property of Sponsor and will not be returned. Sponsor will not be responsible for illegible, late, lost, mutilated, misdirected, postage due or stolen manual entries.

Limit: One (1) entry per person during the Promotion Period regardless of means of entry.

PRIZES: There is one (1) Grand Prize of $15,000 available to be won.

WINNER SELECTION AND NOTIFICATION: Grand Prize winner will be selected on or around March 20, 2010 in a random drawing from among all eligible entries received during the Promotion Period.

The Grand Prize winner will be notified initially by e-mail and confirmed in writing. Winner will be required to complete and return a notarized Affidavit of Eligibility and Liability Release (the "Affidavit") and, except where prohibited by law, a Publicity Release within seven (7) days of the date of initial notification. Failure to respond to any notification, return of any notification as undeliverable, or failure to return these documents within the required time period may result in disqualification and selection of an alternate winner. If Sponsor so elects, the potential prizewinner may be required to submit to a confidential background check to confirm eligibility and help ensure that the use of any such person in advertising or publicity for the Sweepstakes will not bring Sponsor into public disrepute, contempt, scandal or ridicule or reflect unfavorably on the Sweepstakes as determined by Sponsor in its sole discretion. Sponsor reserves the right to disqualify a potential prizewinner on the basis of the findings resulting from such background check. Winner also may be required to complete IRS tax forms as requested by Sponsor and to provide proper photo identification to be returned with the Affidavit or the prize may be forfeited.

Odds of winning will depend on the total number of eligible entries received.

PRIZE CONDITIONS: Any winner's entry and acceptance of a prize awarded constitutes permission for the Sponsor to use said winner's name, photograph, likeness, statements, biographical information, voice and address (city and state), in all forms of media, in perpetuity, without notice or further compensation, except where prohibited. All federal, state, or other tax liabilities, if applicable, are the sole responsibility of the prizewinner. If any prize is returned as undeliverable, this will result in disqualification and an alternate winner will be selected from among all remaining eligible entries.

TERMS AND CONDITIONS: This Sweepstakes is subject to all applicable federal, state, and local laws. By participating, entrants agree to be bound by these Official Rules and the decisions of the Sponsor and Independent Sweepstakes Administrator, and waive any right to claim ambiguity in regard to the sweepstakes mechanics, and/or the interpretation of these Official Rules. LIMITATIONS OF LIABILITY: Incomplete, corrupted or untimely entries are void and will be disqualified. Sponsor and its agencies are not responsible for incomplete, corrupted or untimely entries or for incomplete, lost, interrupted or unavailable network, server or other connections, miscommunications, computer or software malfunctions or telephone transmission problems or technical failures, garbled transmissions, damage to a user's computer equipment (software or hardware) or other errors or malfunctions of any kind whether human, mechanical, electronic or otherwise. Proof of sending or submission will not be deemed to be proof of receipt by Sponsor. The Web site's database clock will be the official timekeeper for this Sweepstakes. Sponsor reserves the right at its sole discretion to modify or cancel this Sweepstakes at any time for any reason without notice or obligation. Sponsor, its affiliate companies, parents, subsidiaries, officers, directors, and agents are not responsible for any incorrect or inaccurate information, including, without limitation, that caused by Web site users or by any of the equipment or programming associated with or utilized in the Sweepstakes or by any technical or human error which may occur in the processing of submissions in the Sweepstakes. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries.
